A decision has been made to merge Gade Surgery and Chorleywood Health Centre to form one GP practice, effective from 2nd June 2025.
All patients registered with these practices should have been contacted directly.

This includes notice of some temporary closures needed to support the merger, some impact to prescriptions, registrations and online services.
