If you were registered as a Chorleywood Health Centre patient before the merger, we are sorry that some patients are experiencing disruptions to their online access to medical records. This is because the Chorleywood Health Centre patients are being moved over to the merged practice’ systems.
To restore your access using the NHS App or NHS.UK, please visit: https://help.login.nhs.uk/setupnhslogin/
To restore your access using the Patient Access app, you will need to visit the practice to complete a form and provide ID. We understand how important this service is, and we sincerely apologise for the inconvenience.
