New Patient Triage System Launching 6th October

From 6th October, Gade and Chorleywood Health Centres will be introducing a new patient triage system designed to improve access, streamline care, and ensure patients are directed to the most appropriate healthcare professional as quickly as possible.

Under the new system, all patients will be asked to complete a short online form when requesting a GP appointment. This form helps the clinical team assess the urgency and nature of the request, allowing for more efficient and effective care.

For patients who are unable to complete the form themselves, the reception team will be on hand to assist either in person or over the phone.
